Aftercare

The Aftercare Program provides young people who have completed the residential programs at Triple Care Farm one-on-one support and guidance as they return to the community.

Aftercare

Young people are offered up to six months of support in the Aftercare Program once they complete Withdrawal and Rehabilitation programs. An Aftercare Worker keeps in touch and is there to offer practical advice and be a mentor as the young person transitions back into the community.

This is an important part of treatment and gives young people the best chance to maintain the positive changes they have made at the farm.

Aftercare workers support the young people as they re-engage with their family and re-engage with pathways to education and employment.

A past student, Isabel came to Triple Care Farm Detox having tried to get off drugs before, “I just need help to detox so that I can get back to school but I can’t do it on my own and I’m scared to go anywhere else” she said.
